Membership Details:
Radiance Pediatrics is a monthly membership service.
Payment is made directly from the patient/family to the provider via an online program set up at time of enrollment. Radiance Pediatrics does not bill insurance nor medical share programs.
By law, Patients with Medicaid Must Sign a Direct Pay Agreement To be a part of Radiance Pediatrics.
Monthly Membership Fees*:
0-2 years - $200
2-15 years - $125
16-26 years - $100
One Time Registration fee: $249 per child
Discounts available: 3 or more enrolled children; foster care children; full annual payment.

20-mile radius effective January 2023
**Distance Travel Fees apply
$15 per trip outside a 10-mile Radius
$25 per trip outside a 15-mile Radius
$35 per trip outside a 20-Mile Radius
New families outside of the service area will be seen at the Urban Family Co-op by appointment only.
